Under the Safety, Health and Welfare at Work Act 2005, employers must report accidents as may be prescribed. Under Part X of the Safety, Health and Welfare at Work (General Applications) 1993, employers are required to forward details of certain reportable accidents and dangerous occurrences to the HSA.
We can assist with completing notification forms IR1 (notification of accidents) and IR3 (notification of dangerous occurrence) and speak on your behalf during subsequent enquiries by the HSA.
We can also conduct a full investigation of events, after which you will receive a report of our findings along with recommendation for preventing a recurrence. The report you receive will give details of immediate causes along with the more important root causes of the event. This means the very many potential outcomes of these failures can be avoided, thereby giving greater reductions in the likelihood of this or other potential outcomes.
